MOUNT PULASKI -- Olivia A. Polk, 74, of Mount Pulaski died at 4:47 p.m. on Monday, Sept. 19, 2005, at Memorial Medical Center in Springfield.

Her funeral will be at 1 p.m. on Thursday at Fricke-Calvert-Schrader Funeral Home in Mount Pulaski. Bill Shanle will preside.

Mrs. Polk had been a programmer and supervisor at Lincoln Developmental Center in Lincoln.

She was born July 28, 1931, in Sturgis, Ky., to Carl M. and Kate Collins O'Leary. She married Thomas Polk on Oct. 9, 1949, at Grove Center, Ky.

She is survived by her husband, Thomas Polk Sr.; three sons, Thomas Jr. (and Margie) Polk of Mount Pulaski, Tim (and Lou Ann) Polk of Mount Pulaski and Terry (and Dianne) Polk of Decatur; one brother, Daniel (and Marnell) Taylor O'Leary of Sturgis, Ky.; seven grandchildren; and three great-grandchildren.

She was preceded in death by her parents and one brother.

She was a member of Sturgis Christian Church in Kentucky.

Memorials may be made to the American Diabetes Association.
